Hormonal Birth Control Methods

What it is: Daily oral medication containing estrogen and progestin (combined pills) or progestin alone.

Benefits:

  • Easily stopped if pregnancy is desired


Considerations:

Emergency Contraception

Emergency contraception (EC) includes methods used after unprotected intercourse or contraceptive failure to reduce the risk of pregnancy. EC does not terminate an existing pregnancy.

How it is used: Taken as a single oral dose as soon as possible after unprotected intercourse.

Effectiveness & timing: Most effective when taken within 72 hours (3 days), but may offer some benefit up to 5 days.

Benefits:

  • Widely available over the counter
  • No prescription required
  • Safe for most patients


Considerations:

  • Less effective with increasing time after intercourse
  • May be less effective at higher body weights
  • Does not provide ongoing contraception


May be best for:

  • Patients needing quick, immediate access
  • Recent unprotected intercourse within 3 days

How it is used: Taken as a single oral tablet by prescription.

Effectiveness & timing: Effective up to 120 hours (5 days) after unprotected intercourse and maintains consistent effectiveness throughout that window.

Benefits:

  • More effective than levonorgestrel pills, especially later in the 5-day window
  • Effective at a wider range of body weights


Considerations:

  • Requires a prescription
  • Should not be combined with hormonal contraception immediately after use


May be best for:

  • Patients presenting 3–5 days after unprotected intercourse
  • Patients with higher body weight where levonorgestrel may be less effective

How it is used: Inserted into the uterus by a clinician within 5 days of unprotected intercourse.

Effectiveness & timing: The most effective form of emergency contraception, with over 99% effectiveness.

Important note: Emergency contraception is most effective when used as soon as possible. It does not protect against future unprotected intercourse. Ongoing contraception should be discussed after EC use.

Long-Acting Reversible Contraception (LARC)

What it is: A small device placed in the uterus that provides long-term pregnancy prevention.

Benefits:

  • Over 99% effective
  • Long-lasting and reversible
  • Minimal daily effort

Hormonal IUDs

How they work: Release a small amount of progestin locally in the uterus.

Benefits:

Non-Hormonal IUD (Paragard®)

What it is: Paragard® is a non-hormonal intrauterine device made of copper that is placed in the uterus to prevent pregnancy. It does not contain estrogen or progestin.

How it works: The copper creates a local inflammatory response in the uterus that prevents sperm from fertilizing an egg. It begins working immediately after placement.

Benefits:

  • Hormone-free contraception
  • Over 99% effective
  • Longest-lasting reversible method (effective for up to 10–12 years)
  • Can be used as emergency contraception if placed within 5 days of unprotected intercourse
  • Rapid return to fertility after removal
